Patient carer
We support individuals with advanced, progressive, incurable illness to live as well as possible until they die. Our work encompasses the supportive and palliative care needed by both patient and family throughout the last phase of life and into bereavement. A key element of this is to ensure patients die in their place of choice - which is usually at home and outside the hospital.
We also hold training sessions on anticipatory planning, and on how to discuss end of life with patients and families. Our work has increased, (+130%), the number of patients receiving the care described in the Gold Standard Framework (GSF) in particular for non-cancer patients.
