Our LTC management is a pre-emptive, coordinated approach to the ongoing care of a targeted group of patients with chronic conditions. We aim to reduce unplanned care by supporting patients to manage their own conditions well. At the heart of what we do is monthly risk-stratification of all patients on our GPs’ lists using multiple data feeds. Once identified at high risk of hospital readmission, the lead GP, together with a multidisciplinary team, conducts a detailed review of the patient and discusses targeted, proactive management. In each case, a lead clinician is put in charge of case managing the patient that has been identified as being at high risk.
