Risk Stratification Tool

Our risk stratifier is a customised version of the Combined Predictive Model developed by the King’s Fund and validated on the UK population. We have programmed the model’s algorithm and the required data extraction queries, customised it for our patient population, and tailored it to the needs of the care providers using it. The model improves predictive accuracy through combining historic secondary care data (e.g. inpatient data) with ‘real-time’ GP electronic patient records. Legacy systems generally utilise historic secondary care data only.
Review and Care Planning
Every month we run the model and those above a certain risk threshold undergo a detailed review and care planning by a multi disciplinary team including social and mental health staff. All high-risk patients are assigned a lead clinician who acts as the central point for the patient for his/her care’s coordination. These patients are then actively case managed. This hands-on approach has been instrumental in reducing the risk scores of patients, thereby keeping them well managed closer to home.
